4. Citizenship/Place of Incorporation
Please indicate the applicant's State/Country of incorporation. If the applicant is an individual, then please provide the country of citizenship.

Enter the applicant's fax number in the box below.

10. Trademark Information
If the mark you wish to register consists of only words, letters and/or numbers, with no claim to any particular design, font, style, size or color, then please enter the name of each trademark you are seeking to register in the box below.

11. Description of the mark
Please enter a complete and accurate description of the overall mark in the box below.
12. Significance of the mark
If any word, letter or number that is part of the mark has a significance in the relevant trade or industry or as applied to applicant's goods or services, it is advisable to clarify this on the application to avoid questioning by the examining attorney during the application review process. 13. English translation of the mark
Please provide the English translation of any foreign wording in the mark in the box below.
14. Good and Services Information
Please described in detail all the goods and/or services provided or to be provided under the mark.
15. Is this a U.S. Federal or Florida trademark application?
Is applicant seeking to register the mark federally with the United States Patent and Trademark Office or with the Florida Secretary of State for use only in Florida?
16. Filing basis for application.
 Use In Commerce: The mark is being used in interstate commerce between more than one State, or in commerce between the U.S. and a foreign country, in connection with ALL the goods and/or services listed in Item 14 above.

 Intent to Use: The applicant has a good faith intention to start to use the mark in commerce at a later date after filing the application.

 Foreign Application: U.S. federal application must be filed within six (6) months of filing a trademark application for the exact mark in a country that is a party to a trademark treaty with the United States. The U.S. application will receive a priority filing date matching the previously filed foreign application.

 Foreign Registration: U.S. federal application must be for the exact mark as the foreign registration currently in force in a country that is a party to a trademark treaty with the United States.

17. For “Use in Commerce” Applications Only.
Please provide the date (month, day, year) of first use anywhere. The date of first use anywhere is the date the goods were first sold or transported or the services were first provided under the mark even if the sale was local.

Please provide a .jpg image file from your local drive showing the mark as used in commerce in connection with the goods and services listed in Item 14 above. For goods, acceptable specimens are scanned or digitally photographed tags, labels, instruction manuals, or containers that show the mark on the goods or packaging for the goods. For services, acceptable specimens are signs, photographs, brochures or ads showing the mark being used in the sale or advertisement of the services.

18. For Applications based on a Foreign Application Only.
Please provide the application filing date (month, day, year), the application number and the name of the country where the application was filed in the box below.
19. For Applications based on a Foreign Registration Only.
Please provide the registration date (month, day, year), the registration number and the name of the country where the registration was issued in the box below.


Also, please provide a .jpg image file of a copy of the foreign registration certificate. If the foreign registration is not in English, you will also need to provide a .jpg image file of the translation of the foreign registration. The translator must sign the translation but does not need to swear to the translation. If these documents are not available now, they will have to be provided during the application review process.

20. Person who will sign application
Please enter below the full name, title and phone number of the person who will sign the trademark application on behalf of the applicant. For instance, if the applicant is a corporation, then one of the corporate officers signs the application. If the applicant is a limited liability company, the managing member signs the application.
